Sign In Sheet and Liability Waiver

This document releases the Sierra Club and its leaders from liability for events that happen on an outing. By signing a liability waiver, the participant acknowledges the risks involved in the outing or activity, assumes those risks, and agrees to not hold the Sierra Club and its leaders responsible for injuries and/or death related to the outing or activity.

There are several versions of this form:

• Angeles Chapter's list version is used for most outings within the chapter. It allows multiple sign-ins per page, and asks participants to provide emergency information that may prove useful. Use of this version is strongly recommended for adults participating in outings that take participants to any location where access to emergency assistance may be limited, but it may not be used for minors.

• The Individual Liability Waiver is the single-person version of the form. Minors must use this version, but because it doesn't collect any emergency information, it is not recommended for adults, particularly if the outing takes participants to any location where access to emergency assistance may be limited.

• Angeles Chapter entities who wish to create their own custom form may do so in compliance with national guidelines regarding Modification of Forms. Specifically, the wording of the liability waiver may not be changed, and all forms must include the "Name," "Signature," and "Early Sign Out" information for each participant.

Medical Treatment Authorization and Consent Form

Chapter safety policy requires that minors have available an Authorization and Consent for Medical Treatment for use by the Trip Leader in the event of injury or illness. (PDF document)

Incident Report Form

Complete this form to report injuries or illnesses on outings. Send it into the Regional Outings Department as soon as possible. (MS Word document, PDF document)

Patient Report Form

This form is used during an incident to assist leaders trained in first aid to assess patients with serious injuries or illness. It will also communicate the status and history of a patient to medical and rescue personnel not present during the incident. Always carry at least two copies so that one can be carried by somebody sent to get help. (PDF document)
