Trip Report - LEADERS: Your outing is not covered by the Sierra Club Liability insurance until a Trip Report
Form has been submitted via email or regular mail to Tina Reimer
(ICO Treasurer) promptly after your outing is completed.

Incident Report - LEADERS: Any injury or illness that occurs during an outing must be reported via this form. Follow
the instructions on the form for where to send it. Also please send a copy to Tina Reimer
(ICO Treasurer). Leaders should carry two blank copies of this form with them on all outings.
